New Patient Information
If it’s your first time visiting the website, we want to start by saying welcome to Cross Creek Dental Care! We want to make your next dental visit a comfortable and productive one, no matter how simple or complex your treatment needs may be. On our new patient page, we’ve included everything you need to become a part of our dental family, including new patient forms, what to expect during your first visit, and methods for financing your care.
Your First Visit
As you arrive for your first visit, we’ll ask you to take a seat in our waiting room for a few minutes before calling you into our dental office. If you haven’t already, you’ll need to fill out a new patient form and hand it to one of our front desk team members. Once we call you in, Dr. Leon will perform a brief exam of your mouth to confirm if any treatment is needed. With time permitting, we may also perform a dental cleaning. If not, we’ll ask you to schedule the cleaning at a later date.
New Patient Forms
Getting to know you and your family is incredibly important to ensure that we provide the highest quality care possible. That’s why it’s very important that you fill out our new patient forms either once you check in at our dental office or on your own time at home. You can download these forms and print them out on your own time for your convenience. Once you’ve filled them out, simply bring them with you to our dental office and hand them to our front desk before waiting to be seen.
